Time since having a high honos item

A useful analysis tool, when trying to manage current caseloads. Not helpful for outcomes.

Counts the number of days (from current) since the person has had a rating on a honos item of 3 or 4

This is possibly an example of what I was just talking about @milanwiedemann - a function that will run fine in R but which would be nice to have in the SQL backend so we can, e.g. filter by it.

(not sure if you're creating a SQL-related issue, I would link to that if you had, but I'll leave that for you to decide)
