User can 'create request' to ask for help

Acceptance criteria

• User can see the list of fields on a create request *per mockup below
• User can fill out the fields easily (Keyboard pops up and down etc.)
• User can hit 'create request' button and it saves the request to the db
• User can hit the 'back button' to get back to home or to their previous page

We're probably going to revisit this page based on feedback so mostly this is just to lay the ground work for other work. Specifically we need create request in order to have a request in order that we can then have comments on it.

Assume that User has already been navigated to the create request page - #70

Out of scope - for now

• Formatting
• Markdown, 'at' @ tagging of other users, links, attachments, emoji etc etc

We might revisit this after we have done the comment user story.
