Description

As a user, I want collect feedback about mitigation and preparedness actions activated so that i can monitor, plan and decide what next needed to be performed.

  • I should be able to issue a simple survey(baseline) to collect needed information from wards, sub-wards and other prone areas
  • I should be able to collect feedback from community leaders around wards, sub-wards and other prone areas

Web UI:

  • I should be able to view response from the conducted survey
  • I should be able to view feedback associated with activated actions
