Document the requirement of a note type called "Basic", with fields "Front" and "Back"

A few people, including me, run into the cannot create note because it's empty problem (see issue #19, or issue #50).

As suggested in the former issue, one of the suspected reasons for this problem is that the plugin assumes the Anki collection to have a note type called Basic, with fields named Front and Back. This note type exists in the English version of anki, but users who use different language settings in anki may not have that exact note type, as the names of the two fields will differ according the language chosen.

It's easy to fix, if one knows about it. I don't see a need to change the implementation of the plugin, but a remark in the documentation would be very useful.
