Supplementary material for the decisions of SOFA

Hi, many thanks for this important resource--code and data.

In the article associated with this repository, I read this:

A more detailed comparison of the severity scores is provided in the supplementary material, along with a discussion of the assumptions made in calculating the scores.

But I couldn't find it there or in this repository! I thought the authors might like to post that here. My apologies if I missed it.

Also, is this an appropriate place to post questions/issues with the current score implementations? Thanks!
