Enable the extension to work in local mode

Is your feature request related to a problem? Please describe.

It would be great if the extension could work in with any potential Qiskit Code Assistant LLM running on local. I could imagine we release an Open Source model so we could run it locally using the API exposed by any existing solution such as ollama, Jan, etc

Support, apart from the Qiskit Transpiler Service, to connect to other APIs running on local that follow the OpenAI API schema https://platform.openai.com/docs/api-reference/introduction
