The server should be configured to limit

Number of Connections/Sessions per IP
Number of Connections/Sessions per Username
Number of Subscriptions per Session
Number of pending Messages per Session
Duration of inactive/disconnected Session

The server should also be able to set rate limits on

Control packets per second
PUBLISH per second
Bytes per second

Ideally this should all be set by the auth plugin
