Update calls to entity-fishing API

The update of the API should not be forgotten ;-)

Instead of just calling /disambiguate, you call getConcept for each entity reference (Wikipedia/wikidataID) in case it needs to be retrieved.

This should also reduce the amount of traffic as such information can be cached and reused for repeated entities (e.g. likely to happear in the same document)
