Anchor of titles of lists #250

When user makes a cross-reference to a list having a title, the anchor is published after the title.

As a consequence, when user clicks on the hyperlink the title of the list is out of sight :

test case with result as of today with DocBook XSL 1.75.2 (I checked and DocBook XSL 1.78.1 looks the same at lines 64 and 76)
