Current practice is to put the stem.so file in a different place for each php version.
Would it be better to create a directory /usr/local/lib/php-stem and always put it there.
Then put that location in stem.ini

Great, if a single location (as you suggest) certainly seems simpler/easier.
Ideally keeping the PHP version and critical desiderata visible, e.g. in the filenames like php7.4-stem.aarch64.tar or such, with a symlink or whatever? But those are small details I'm sure you've got under control.
