Remove need for cohort extractor to access latest VMP mapping from the secure environment #2101

Cohort extractor currently accesses Open Codelists from the secure environments to get the latest VMP mapping (code).

This is run weekly on the secure environments to update the mapping data in the database.

To avoid the secure environments needing to contact this external service, we should look into other ways to get that data onto the server.

This is to address risk 48 in the security risk register.
