Break out a person name from the set under a person name authority

If a user makes a mistake related to a name and that name there are
currently 2 possiblities

1.  The name should be in a brand new name authority set or
2.  The name should be combined into a different name authority set

There must be a way for a user to correct this rather than having the admin
correct it.

If the name is attached to a publication, the user cannot remove it from
their own set of names - this need to be fixed at a minimum.
