default_exception_handler doesn't work

The self.default_exception_handler method in the application controller never calls. Looks like RescueRegistry::Context.class_methods call overrides that method.

@bguban Unfortunately, I'm at a job where I no longer work with Ruby or Rails. I'm happy to accept PRs but won't be doing any of the work myself.
