Update README to make URLs clickable #333

At a minimum, all links should be clickable. I took the liberty of working the links into the wording, but it could also just be done by just linkifying all the URLs with the `[link_text](url)` syntax.

Actually, I've just confused myself as it seems the README already has the basic linked URLs. I'm completely confused now, so if this is confusing to you as well, feel free to just close this |
