Frontend: make necessary changes for survey question logic

We're still gathering info on what needs to be done here. Open questions:

• How to store non-question text (intro/thank you), do we show these to public users?
  • Probably needs to live alongside the questions in the database so that they're changeable in the same place
• How to handle questions that are conditional on the responses to other questions
• Does the answer format need to capture what paths were taken in a conditional question?
• Are "bonus" questions logically different than normal questions? Do we show these to public users?
