1.3 Maintenance Release

This is a maintenance release of the 1.3 CQL translator that includes the following fixes:

#482: Updated the type of extension.url to FHIR.uri
#484: Fixed MoneyQuantity in the FHIR model info
#486: Fixed date range optimization not functioning with choice types
#554: Add null tests to certain timing phrases
